서론: 문맥압항진증의 합병증인 큰비신단락(Large spleno-renal shunt)의 치료방법으로 방사선 중재적 시술을 통하여 큰비신단락(Large spleno-renal shunt)으로의 문맥혈 유실을 방지하기 위해 변형스텐트와 플러그를 이용하여 문맥압을 감압시키는 혈관색전술의 유용성에 대해 보고하고자 하였다.
대상 및 방법: 간경변으로 인해 간이식 수술을 하였던 환자로 Spleno-renal shunt에 의해 문맥압이 높아진 환자를 대상으로 시술 하였다.
결과: 큰비신단락(Large spleno-renal shunt)에 Hercules vascular stent(S&G BIOTECH INC.)24x60mm와 Amplatzer vascular plug4 device(St. Jude Medical)-8mm를 설치하였으며, 그 결과 Follow up portography에서 Spleno-renal shunt로의 혈류(Blood flow)는 관찰되지 않음을 확인한 후 시술을 종료하였고 시술 중 시술과 관련된 합병증은 관찰되지 않음을 확인하였다.
결론: 간문맥항진증의 이차적 합병증인 큰비신단락(Large spleno-renal shunt)을 치료하기 위한 방편으로 정맥류 및 단락 부위의 직접결찰 등 수술적 방법이 아닌 비침습적인 방사선중재적 인터벤션시술을 통하여 환자의 고통감소 측면과 수술의 위험성에서 조금 더 부담을 줄일 수 있었고, 방사선중재적 시술방법이 큰비신단락(Large spleno-renal shunt)치료에 있어 통상적인 방법이나 최종적인 치료방법은 아니지만, 큰혈류(Large blood flow)를 막아야 하는 유사한 상황에서 적절한 색전물질(Embolic material)을 찾기 어려울 때, 혈관계질환의 치료에 이용되는 스텐트와 플러그를 이용하여 시술함으로써 비교적 간단히 효과를 얻을 수 있는 치료방법으로 사료된다.
